In May 1989, Chris Johnstone, a junior doctor in the NHS, was driving down a road in Scotland, having managed to take a rare break from his slavish hospital grind, a role which by law at the time required him to work a maximum of 83 hours a week. More often, though, this figure ran to well over 100 hours.
