Tribunals and inquiries no substitute for accountability

It’s time to change the way we deal with scandals and start making individuals answerable for their behaviour – with real penalties

The HSE has a lengthy and well-documented problem with accountability.

In 2014, the Health Information and Quality Authority (Hiqa) was commissioned to investigate maternity services at the Midland Regional Hospital in Portlaoise. This followed revelations by RTÉ’s Prime Time Investigates that four full-term babies died between 2006 and 2013 shortly after or during birth because of a shortage of oxygen.
