A technical failure at the HSE during the Covid-19 pandemic led to reduced security for more than a million people, the agency has admitted.

The HSE has confirmed that a “misconfiguration” in its Covax database - which stored information concerning Covid-19 vaccines - occurred, which was flagged by an external expert in 2021.
