HSE admits risk of not learning from past errors is huge threat to body

The HSE has identified the risk of not learning from its past health scandals as one of the biggest threats to the organisation.

It comes as it attempts to resolve a row with the health watchdog Hiqa over a highly critical draft report into maternity services in Portlaoise Hospital.

The risk of failing to learn the lessons from “serious adverse events” was one of the 16 critical issues flagged up in its new “risk register”,...
