Government fails to act on child death report
The government has implemented only one of the key recommendations made in the child death review report of 2012. The harrowing report documented the deaths of 196 children under state care.
Key recommendations included the setting up of an independent child death review group. Independent reviews into the deaths of ten children were singled out as another top priority by the Independent Child Death Review Group (ICDRG), which was chaired by child law expert Dr Geoffrey Shannon.
The report’s authors said the government should put aftercare on a statutory footing. However, none of those recommendations has been fully implemented.
