An alarm went off in the Leixlip water treatment plant at 1.10am. The lone plant operator on duty was supposed to respond to this alert about a malfunctioning pump, but never did. The hours ticked by. Millions of litres of potentially contaminated water continued to be pumped out of the plant into the homes of 600,000 people in Dublin, Meath and Kildare.
The problem was only spotted when the plant operator was replaced at the end...
