Hospitals that delay major incident reports could lose legal cover

The State Claims Agency (SCA) threatened to withdraw legal cover for hospitals which were failing to report serious incidents on time.

There is a national incident management reporting system, with which health managers are supposed to report any incidents causing harm or death within 24 hours.

But they only reported 27 per cent of these incidents within this timeframe to the State Claims Agency last year.
