A confidential report into the handling of childcare case files by the Health Service Executive (HSE) has uncovered widespread failure to prevent files about vulnerable minors from falling into the wrong hands.

In one case, an entire catalogue of files relating to vulnerable adolescents in the HSE’s Southern region was discarded in a photocopying room without any encryption or proper security.

They contained contact names and PPS numbers of patients under HSE care...
