The Health Service Executive (HSE) is facing a legal challenge on the deaths of troubled children in care after the mother of Tracey Fay launched a civil action against the state last week.

The deaths of more than 200 children are being examined by a special taskforce set up due to concerns about the number of children dying in the care of the state.
