Might I suggest a simple, cost-free, mechanism to help prevent abuse such as was identified in a recent Prime Time investigation: it is to place a legal obligation on any staff member who witnesses such an act of abuse to report it to the relevant authority and that a failure to promptly report such abuse to the nominated authority would render the observer guilty of a criminal offence.
