January 30, 2014 Failures in care at the Maternity Unit in the Midland Regional Hospital, Portlaoise are revealed in an RTÉ investigation called ‘Fatal Failures’. The programme tells the stories of the deaths of four babies over a six-year period. It claims that these babies were alive at the onset of labour but died during labour or within seven days of birth due to what the families regarded as failures in care.
